- Color Headshot (minimum 4"x5" or 5"x7"; not to be returned)
- Children's Miracle Network Donation Details

- Talent Request and TWO copies of music selection (Must be received no later than 5pm on Friday, October 5th, 2007!)
- People's Choice Jar (due Friday, November 9th, 2007)
