Required Application Materials
Must be received no later than 5pm
on Friday, October 5th, 2007!

- Platform Statement Instructions - ONLY 100 WORDS NEEDED!
- Local Contract
- Contestant Release and Indemnity Agreement
- Color Headshot (minimum 4"x5" or 5"x7"; not to be returned)
- Entry Fee Details and Sponsor List
- Official Copy of Grades Transcripts from your School (Must be Signed and Sealed by a School Official)
Other Requirements (due dates as indicated)
- Talent Request and TWO copies of music selection (Must be received no later than 5pm on Friday, October 5th, 2007!)
- People's Choice Jar (due Friday, November 9th, 2007)

- Additional Scholarship Opportunity: Advertisement Sales for Program Book & Contestant Incentives (due no later than Friday, October 5th, 2007)
